Treatment of the fractured tip

Decision for the treatment of the fractured tip To remove the rest of an implant is very difficult and very traumatic procedure. Therefore, to leave it in the bone is recommended. The implant is made of Titanium, which is biocompatible. It is almost same as surgical fixation. In case of surgical fixation, the surgical screws are sometimes left in the bone. To remove the tip, it should be done first to remove all the bone around the tip. And then remove tip by a hemostat. Referring to oral surgeons may be needed.

Removal of a fractured tip

1. Using the trephine bur
The trephine bur, which we use mainly in order to eliminate a dental implant or to collect bones for autografting, can be used efficiently to get rid of the remnant orthodontic implant that remains within the bone as being broken.
It is possible to eliminate the implant tip successfully if we reveal the operated part sufficiently by incising soft tissue just above the implant that needs eliminating up to the periosteum, put the trephine bur to the point that corresponds with the implant direction, remove the bone with sufficient depth by running the drill, detach trephine from the bone, fracture the implant and the bone using apparatuses with the sharp tip such as the root picker, and raise this with the pincette.

2. Using the fissure bur and the hemostat(figure 6)

Step 1) Make a flap
A full thickness flap operation should be performed to secure the visual fields.

Step 2) Removing the bone around the fractured tip
Using the thinnest fissure bur, more than a half of the fractured tip should be exposed.

Step 3) Removing the tip
Hold more than a half of the remnant tip firmly with the hemostat or the beak of the needle holder, and then take it out by turning gradually reversely to the direction of insertion.

Step 4) Finishing
After removal of the tip, wash the surrounding parts cleanly with physiological saline and make sure that there aren't any metal remnants. After that, suture the flap.
